Can I fail a DOT physical?

What blood pressure do I need to pass a DOT physical?

Accepted Answer

Your blood pressure must be below 140/90 to receive a standard 1 or 2 year certificate. If your blood pressure is between 140–159/90–99 (Stage 1) at your first exam with no prior history of high blood pressure you can still be certified for up to 1 year. If your blood pressure is 160–179/100–109 (Stage 2) at your first exam you may receive a one-time 3-month certificate to allow time to get it under control.

My blood pressure was high at my last DOT exam. What happens this time?

It depends on how high it was previously and what it is today. If your blood pressure was previously Stage 3 (180/110 or higher) and is now below 140/90 you can be certified but only for up to 6 months. If it was previously Stage 2 and is still Stage 2 today you cannot be certified until it is below 140/90.

Can blood pressure medication affect my DOT certification?

Most blood pressure medications are acceptable for DOT certification. However some medications — particularly alpha blockers like doxazosin and centrally acting agents like clonidine — can cause dizziness or sedation that the examiner will assess. Generally if your blood pressure is well controlled on medication and you have no significant side effects you can be certified.

Does sleep apnea disqualify me from getting a DOT medical certificate?

Not automatically. Under the 2024 FMCSA guidelines sleep apnea is evaluated on a case-by-case basis. If your sleep apnea is treated and you have no excessive daytime sleepiness you can be certified. The key question the examiner asks is whether you are currently sleepy or impaired — not simply whether you have a diagnosis.

Do I need to show my CPAP compliance data at my DOT physical?

Under the 2024 FMCSA Medical Examiner's Handbook CPAP compliance data is no longer a mandatory requirement for certification. The previous requirement for documented CPAP compliance was a 2016 standard that is no longer in effect. Your examiner will assess your current symptoms and daytime alertness. Compliance records are recommended but do not gate your certification under current guidelines.

I have sleep apnea but I don't use a CPAP. Can I still get certified?

Possibly. If you are asymptomatic — meaning you are not experiencing excessive daytime sleepiness or falling asleep while driving — you may still be certifiable under 2024 guidelines. If your sleep apnea is mild (AHI less than 15) you may receive a 1-year certificate. If it is moderate to severe (AHI 15 or higher) without treatment your examiner may issue a shorter 3-month certificate and strongly recommend you begin treatment.

I was just diagnosed with sleep apnea. Can I still drive commercially while I get set up on CPAP?

Under 2024 guidelines there is no mandatory waiting period before certification while initiating CPAP. If you are not currently symptomatic your examiner can certify you while you begin treatment. Your examiner will use clinical judgment to assess your current level of alertness and symptoms.

I have Type 2 diabetes on metformin. Will I pass my DOT physical?

Most drivers with Type 2 diabetes controlled on oral medications like metformin pass their DOT physical without issue. The main concerns are whether you have had any severe hypoglycemic episodes requiring help from another person, whether you have significant peripheral neuropathy affecting your feet or legs, and whether you have severe diabetic eye disease. If none of these apply and your diabetes is well managed you should be certifiable for up to 1 year.

I use insulin. Can I still get a DOT medical certificate?

Yes — but only through the FMCSA Insulin-Treated Diabetes Mellitus (ITDM) Exemption Program. You must apply directly to FMCSA for an exemption, carry the exemption certificate with you at all times while driving, and bring a completed MCSA-5870 form signed by your treating provider within 45 days of your exam and 3 months of blood glucose monitoring records to your DOT physical. Without a valid exemption on file an insulin-treated driver cannot be certified.

What is the MCSA-5870 form?

The MCSA-5870 is the Insulin-Treated Diabetes Mellitus Assessment Form. It must be completed and signed by your treating provider — your endocrinologist or the doctor managing your diabetes — and brought to your DOT physical. It must be dated within 45 days of your exam. It confirms that your treating provider has assessed you as stable, compliant with monitoring, and fit to safely operate a commercial vehicle.

I take Ozempic or another GLP-1 medication. Does that affect my DOT physical?

Accepted Answer

GLP-1 medications like semaglutide (Ozempic) and liraglutide (Victoza) do not cause hypoglycemia on their own and have no CNS effects that would affect certification. They are generally acceptable for DOT purposes. If you take a GLP-1 in combination with a sulfonylurea or insulin the hypoglycemia risk from those other medications still applies.

I take Suboxone (buprenorphine) for opioid use disorder. Can I get a DOT physical?

Under the 2024 FMCSA Medical Examiner's Handbook buprenorphine and methadone are no longer automatic disqualifiers when prescribed for opioid use disorder treatment. Your examiner will assess whether your dose is stable, whether you are experiencing any sedation or impairment, and whether your prescribing provider can attest to your fitness for CMV operation. A clearance letter from your prescribing provider is required.

I take methadone at a clinic. Will that disqualify me?

Not automatically under 2024 guidelines. Methadone for opioid use disorder treatment is now evaluated case-by-case. The examiner will assess stability of your dose, whether you experience sedation during working hours, and whether your treatment provider can provide documentation of your fitness for commercial driving.

I take Chantix (varenicline) to quit smoking. Is that okay for my DOT physical?

Accepted Answer

Varenicline has a history of neuropsychiatric side effects including depression, agitation, and abnormal dreams. Your examiner will ask whether you are experiencing any of these side effects. If you are asymptomatic on varenicline it is generally not disqualifying. If you are experiencing mood changes or agitation your examiner will need to assess more carefully.

I take CBD oil. Will that affect my DOT drug test?

Potentially yes. CBD products are unregulated and frequently contaminated with THC. A positive DOT drug test for THC while using CBD is treated exactly the same as intentional marijuana use and is disqualifying. Federal law governs DOT drug testing — state cannabis laws do not apply.

I take a sleeping pill occasionally. Will that affect my DOT physical?

Sleep medications like zolpidem (Ambien) can cause significant next-day impairment. Your examiner will ask about your use. If you take sleep medication within 8 hours of driving or on a regular nightly basis your examiner will assess whether this creates a safety concern.

I take lithium for bipolar disorder. Can I still get certified?

Lithium is not automatically disqualifying. Your examiner will want to see psychiatrist clearance confirming your mood has been stable for at least 12 months, your lithium levels are within therapeutic range, and the medication is not causing cognitive or psychomotor side effects at your current dose.

I had a seizure years ago. Can I drive commercially?

It depends on how long ago it was, what caused it, and whether you are on medication. For a single unprovoked seizure you must be seizure-free for at least 1 year off anticonvulsant medication or 5 years on medication. For diagnosed epilepsy (2 or more unprovoked seizures) the requirement is 5 years seizure-free whether on or off medication. A neurologist clearance letter is required in all cases.

I had a TIA (mini stroke). Can I still get a DOT physical?

Possibly. You must have a complete neurological workup identifying and treating the underlying cause, no residual neurological deficits, and be symptom-free for at least 1 year. Neurology clearance is required.

I have migraines. Will that disqualify me?

Accepted Answer

Most drivers with well-controlled episodic migraines can be certified without a specialist letter if your migraines are infrequent, you typically have warning before an attack, and your medications are safe for driving. However certain migraine types are disqualifying — including hemiplegic migraine with motor weakness, basilar-type migraine with brainstem symptoms, and any migraine that has caused loss of consciousness. Chronic migraine and vestibular migraine require neurology clearance.

I have Ménière's disease. Can I get a DOT medical certificate?

Ménière's disease is evaluated case-by-case under 2024 FMCSA guidelines. To be considered for certification you must have been free of acute vertigo attacks for at least 12 months, have no history of drop attacks, not be taking sedating vestibular suppressant medications daily, and meet the FMCSA hearing standard. ENT or neurotology clearance is required.

I had a heart attack. Can I still drive commercially?

Possibly. There is a mandatory 3-month waiting period following a heart attack before certification can be considered. After that period you need cardiology clearance confirming stability, adequate exercise tolerance, and your cardiologist's opinion that you are fit for commercial driving. You will be certified for up to 1 year with annual follow-up required.

I have a pacemaker. Does that disqualify me?

Not necessarily. It depends on why the pacemaker was placed. Pacemakers for sinus node dysfunction or AV block require a 1-month waiting period then cardiology clearance. Pacemakers for hypersensitive carotid sinus with syncope require a 3-month waiting period. However pacemakers placed for neurocardiogenic syncope do not support certification under FMCSA standards.

I have an ICD (defibrillator). Can I drive commercially?

It depends on why the ICD was placed. If it was placed for primary prevention and has never fired you may be certifiable after a 3-month waiting period with electrophysiology clearance. If the ICD has fired it is disqualifying under FMCSA standards.

I have atrial fibrillation. Will I pass my DOT physical?

Accepted Answer

AFib is evaluated case-by-case. If your rate is well controlled, you are asymptomatic, you are anticoagulated appropriately, and you have cardiology clearance you can be certified for up to 1 year.

I have PTSD. Can I get a DOT medical certificate?

Accepted Answer

Yes — PTSD is now evaluated case-by-case under the 2024 FMCSA Medical Examiner's Handbook which includes a dedicated section for PTSD. If your symptoms are stable, you are in treatment, and you do not have active intrusive symptoms or dissociation that would affect your driving, you can be certified with psychiatrist or psychologist clearance.

I take an antidepressant. Will that affect my DOT physical?

Most modern antidepressants — SSRIs and SNRIs like sertraline, escitalopram, venlafaxine, and duloxetine — are generally acceptable for DOT certification. Your examiner will assess whether you experience any sedation, dizziness, or cognitive side effects at your current dose.

I have bipolar disorder. Can I drive commercially?

Bipolar disorder is evaluated case-by-case. If your mood has been stable for at least 12 months with no manic or hypomanic episodes, no psychiatric hospitalizations, and your psychiatrist can provide clearance confirming stability and medication tolerability, you may be certifiable. Active manic or hypomanic episodes are disqualifying.

I am in recovery from alcohol use disorder. Can I still get a DOT physical?

Possibly. If you have been in sustained remission for at least 1 year, have no significant liver dysfunction or peripheral neuropathy from prior alcohol use, and can obtain clearance from a substance abuse treatment provider, you may be certifiable. Active alcohol use disorder is disqualifying.

What vision do I need to pass a DOT physical?

Accepted Answer

You must have distant visual acuity of at least 20/40 in each eye with or without corrective lenses, a horizontal field of vision of at least 70 degrees in each eye, and the ability to recognize red, amber, and green traffic signals. If you need glasses or contacts to meet the standard your certificate will include a corrective lenses restriction.

I only have vision in one eye. Can I get a DOT medical certificate?

Not under the standard certification pathway. However the FMCSA Federal Vision Exemption Program provides a pathway for monocular drivers who have demonstrated adaptation to their vision loss. You must apply directly to FMCSA for this exemption — the medical examiner cannot grant it independently.

What hearing standard do I need to pass?

You must be able to perceive a forced whispered voice at 5 feet or greater in at least one ear with or without a hearing aid. If you need a hearing aid to meet this standard your certificate will include a hearing aid required restriction.
